Kofukan Shito-Ryu Invitational Development Tournament

• I will provide my own properly fitted mouth guard.
• I will provide my own WKF-approved mitts - red/blue or white identical equivalent (some pairs will be available to borrow for competitors under 16).
• I understand that the following equipment is highly recommended for juniors and required for teens and adults and I need to provide my own: groin guards (males) and chest protector (females).
• I understand the following equipment is highly recommended: shin pad and foot protectors, body protection, head guard (juniors). Some sets will be available to borrow on the day (under 16 years only, those 16+ must supply their own).
• Long hair must be tied back and no jewelry to be worn.
• I am responsible for my own protective equipment.

This is a development tournament. I confirm that I have NOT:

• Been a member of a recognised karate State or National team, or
• Won a medal in an Open or Elite event (or any recognised event except a development event) at a State/National/International karate tournament.

1. If the Competitor is under 18 years of age this entry form must be completed by a parent/guardian.
2. The person completing this form agrees to the following conditions of entry.

In these conditions, "Competitor" means the person named in this registration form, "the Organiser" means Kofukan Karate Australia Inc, its officers, servants, agents, and affiliated instructors, volunteers and officials, whether or not they are members of Kofukan Karate Australia Inc or another club or association, and "Opponent" means any person competing against the Competitor.
4. I hereby agree to abide by the conditions of entry, the competition rules, the Code of Conduct, policies and regulations of the Organiser.
5. The Organiser enters this contract as the Organiser and as agent for the Competitors’ Opponent.
6. I acknowledge that karate and participation in the tournament carries a real risk of personal injury.
7. I acknowledge and agree that the Organiser shall not be liable whether in tort or contract or otherwise for any loss or damage or injury whatsoever to my person or property whilst participating in or otherwise attending at the tournament, howsoever caused or occurring, including without limiting the foregoing, the negligence, breach of contract, wilful act or default of the Organiser and whether or not such loss or damage or injury to me occurs within or adjacent to any property occupied by the Organiser.
8. I acknowledge and agree that my Opponent shall not be liable whether in tort or contract or otherwise for any loss or damage or injury whatsoever to my person or property whilst participating in or otherwise attending at the tournament, howsoever caused by or occurring including without limiting the foregoing, the negligence, breach of contract, wilful act or default of my opponent and whether or not such loss or damage or injury to me occurs within or adjacent to my property occupied by the Organiser.
9. I authorise the Organiser to contract on my behalf to exclude my liability as an opponent whether such arises in tort or contract or otherwise for any loss or damage or injury to another competitor however caused or occurring including but without limiting the foregoing, the negligence, breach of contract, wilful act, or default of myself.
10. I expressly agree to and do hereby waive any rights that I may have which are in any way inconsistent with the foregoing.
11. I acknowledge and agree that any photographs or videos taken of me/my child whilst participating in the tournament will be available to Kofukan Karate Australia Inc to be used on their websites or in any advertising/promotional material as they see fit.
12. Refund policy: a full refund minus 10% administration fee will be provided for cancellations at least 24 hours before the event. No refund within 24 hours of the event except in exceptional circumstances.
